Adult Care Home Case Management

Adult care home case managers work with adult care homes in the county to improve the overall quality of care for residents who are Medicaid eligible and require heavy care. These residents need hands-on assistance three or more times per week with eating and/or toileting and/or ambulation. The case manger visits with each adult care home that has heavy care residents on a monthly basis to ensure these residents' needs are being met.
